MOBILE SMARTPHONE/DEVICE USE – TERMS AND CONDITIONS

Welcome! You are eligible to receive services, or are already receiving services, from MedZed Physician Services, LLC and MedZed, Inc. (collectively, “MedZed,” “we,” “us” or “our”).  From time to time, MedZed may send SMS messages/text messages (“Texts”) to you to remind you about your appointments with MedZed or other information related to MedZed.  By receiving Texts from MedZed, or by receiving a referral for MedZed services, you agree to be bound by the terms of service set out below (the “Agreement”). Please read this Agreement carefully before signing up to receive MedZed’s Texts. If you cannot agree to this document, please do not sign up to receive Texts from MedZed.  At any time, MedZed may revise and update this Agreement. Your continued receipt of Texts will mean you accept the revised or updated Agreement.  

PROGRAM: MedZed Appointment Notifications 

FREQUENCY: Dependent on alert settings. Number & size of Texts can vary per user. 

PRICING: Message and data rates may apply. 

MOBILE USER OPT-IN: When you speak to a MedZed representative about enrolling in the Appointment Notification program, they will get your verbal approval to participate.  Then you will receive a Text from MedZed that tells you that you are enrolled.  This means that you authorize MedZed to send you Texts, and that you acknowledge that Texts may be sent by an automatic telephone dialing system. We will also explain how you can opt out of getting any future Texts from us. Additionally, by opting in, you acknowledge that you are at least 13 years of age and are either the wireless account subscriber or have the permission of the wireless account subscriber to send and receive Texts under these Terms and Conditions. MOBILE USER OPT-OUT: You may stop getting MedZed Texts at any time.  To do this, please reply “STOP” from your mobile device to any message that you get from MedZed. After opting out, you will not receive any additional messages, unless you opt back in, which you can do at any time. 

MOBILE USER FEES:  MedZed will not charge you anything for sending or receiving Texts.  MSG & Data rates may apply from your applicable carrier, however. No purchase necessary to participate. 

MOBILE HELP: To get help, reply “HELP” to any message you receive. 

U.S. MOBILE CARRIERS SUPPORTED: Verizon Wireless, AT&T, Sprint/Boost/Virgin, T-Mobile®, US Cellular, Carolina West, C-Spire (Cellsouth), Cellcom, nTelos, Google Voice, and the numerous ClearSky/Interop. The wireless carriers are not liable for delayed or undelivered messages and MedZed does not endorse and one specific carrier, or work, directly or indirectly for carriers. 

MOBILE WARRANTY: MedZed will not be liable for any delays in the receipt of any SMS messages connected with our SMS provider. Delivery of SMS messages is subject to effective transmission from your wireless service provider/network operator. 

MOBILE PRIVACY POLICY: MedZed respects your privacy. We will not share or use your mobile number for any other purpose. No protected health information will be transmitted or shared.  Please see our Privacy Terms and Conditions for more information. We will only use the information you provide to transmit Texts to you. We reserve the right, at all times, to disclose any information as necessary to satisfy any law, regulation, or governmental request, to avoid liability, to protect our rights or property. Texts through your wireless carrier/service provider are not secure messages through encryption or similar technology.
