Our Mission

 

There is no shortage of organizations aiming to provide resources and wraparound social supports to high-need Medicaid and dual eligible members. But MedZed stands alone in our dedication to working exclusively with the highest-cost, hardest-to reach members.

 

The ones who have fallen through the cracks of the health system. The ones who have no relationship with a primary care provider, don’t know how to manage their health conditions and all too often end up in the emergency room and the hospital. The ones who are transient or don’t answer the door or just don’t respond to offers of help.

 

These are our people. We go out in-person and find them and build a bond of trust. We meet them where they are and walk alongside them, empowering them to manage their health and related needs and transform their lives.

Our History

MedZed was founded in 2014 to provide an extra layer of support for health plans to find, engage and care for the Medicaid and Medicare members who cannot be reached by traditional outreach and care management.

We have served 50,000+ high-risk, hard-to-reach individuals for 22 health plans in 11 states and delivered nearly 1 million hours of member care via home visits and member calls.

When everything is bad, I kinda shut down…but MedZed wouldn’t let me give up​

When I was applying for housing, they kept encouraging me, keeping me grounded. And they’re not like most.They don’t just get you something and drop you. They follow up, they still call you and check on you, they don’t leave you out to dry.​
