Connect clients to ECM and CS services
In California, MedZed offers Enhanced Care Management (ECM) and Community Supports (CS) services at no cost to eligible Medi-Cal members. MedZed partners with many Community-based Organizations and Services to get high-need individuals all the help they need.
As an ECM provider, we bring an extra layer of support to your Medi-Cal members. Our field staff of Navigators act as lead care managers, working one-on-one, in-person with members to coordinate their health and health-related services.

ECM is intended for the highest-risk, highest-cost Medi-Cal managed care members with the most complex medical and social needs. To be eligible for ECM, members must be enrolled in a Medi-Cal managed care health plan and meet the criteria for an approved Population of Focus (PoF), including:

Adults without Dependent Children/Youth Living with Them Experiencing Homelessness

Homeless Families or Unaccompanied Children/Youth Experiencing Homelessness

Individuals At Risk for Avoidable Hospital or ED Utilization
-
Individuals with Serious Mental Health and/or SUD Needs
-
Children and Youth Enrolled in California Children’s Services (CCS) or CCS Whole Child Model (WCM) with Additional Needs Beyond the CCS Condition

Children and Youth Involved in Child Welfare
