HOW MUCH DOES MEDZED COST?

MedZed services are available at NO cost to you.
It is a FREE benefit provided to you by your health plan.
Health plans partner with MedZed to help you keep you healthy—at home and in your community.

WHAT CAN MEDZED HELP ME WITH?

Coordinating Care & Providers: appointments, reminders, etc. for primary, specialty and behavioral health and dental services.
Social Support: identifying and applying for both local resources and benefit programs: food assistance, transportation assistance, housing, employment, legal aid, etc.
One-on-One Support: your Navigator is your go-to person. They get to know YOU and coordinate everything and make sure your plan is working for you.

HOW DOES CARE FROM MEDZED WORK?

We come to you in-person at your home or a location that is good for you.
We interview you to understand your needs. Then we develop a customized care plan of medical care and social and other supports.
Our team then comes to you regularly to monitor your health, provide direct care, and help you access services.
Your Navigator helps you understand and take the right steps to improve your health: identifying and applying for resources, making and getting to appointments, and supporting you every step of the way.
In between visits we stay in close touch by phone.

WHO ARE MEDZED’S CAREGIVERS?

All of our caregivers are MedZed employees and work together as a team.
We call our field team Navigators because they work one-on-one with you to help you understand and follow your care plan. They are your guide for your path to better health.

HOW DO I KNOW IF I QUALIFY FOR MEDZED SERVICES?

We currently operate in the following states: California, New Jersey, and Maryland.
If you receive Medicaid services in one of those states from one of the following health
plans, you may be eligible.
You can ask your health plan representative directly or contact us for assistance.
