For Health Plans

We locate and engage your members and reintegrate them into primary, specialty and behavioral health care and dental services to close critical care gaps and keep them out of the emergency room and the hospital.

For Community Based Organizations

We support clinics, shelters, food banks, and many other support organizations in meeting your clients’ complex needs.

Compassion in Action

It Begins with Empathy

Our locally-based Navigators have lived experience of many of the same challenges—and how to overcome them. They are deeply passionate about helping others in need.

We Call Them Navigators for a Reason

Our Navigators are creative and persistent at finding members and gaining their trust. Immersed in the fabric of their communities, they are savvy and well-connected at getting their members help.

We’re in It for the Whole Journey

Navigators work one-on-one, in person with their members on a personalized plan of realistic goals that will work for them. They are there for them every step of the way.

"The reason why I show up to work every day it’s because I am passionate about helping others and it fills my heart to know that I’m able to help somebody that is going through a lot of health problems or is suffering from homelessness, or simply just having mental health problems and not sure how to go about it and we're able to help them out and guide them and making sure that they get the help they need. I’m in 100%."
