MedZed Proudly Featured on DHCS PATH Website: Recognizing the Impact of Our Team's Hard Work


At MedZed, we are thrilled to be recognized for our efforts on the California Department of Health Care Services (DHCS) Providing Access and Transforming Health (PATH) website. Being featured as a success story is not only a testament to the dedication and passion of our team members but also highlights the meaningful impact, we have on the communities we serve.

Our mission has always been to help those in need, especially those experiencing housing insecurity and navigating complex health and social challenges. Through collaboration with DHCS and initiatives like PATH, we have made significant strides in improving care management and ensuring our members receive the support they need.

The Power of Partnership

MedZed’s collaboration with DHCS through multiple PATH initiatives has provided us with the resources, tools, and technical assistance to enhance and expand our Enhanced Care Management (ECM) and Community Supports programs. These programs are designed to deliver life-changing care and services to our most vulnerable populations. Here's how the PATH initiative is empowering us:

  • PATH Capacity and Infrastructure Transition, Expansion, and Development (CITED) funding has accelerated the growth of our Community Health Navigator workforce, allowing us to meet the rising needs of our communities.

  • PATH Technical Assistance (TA) Marketplace services have enabled us to improve our population health management systems, allowing us to serve more members effectively.

  • As participants in PATH Collaborative Planning and Implementation (CPI) across California, we are gaining invaluable insights, building relationships, and learning best practices for Medi-Cal transformation.

A Story of Hope: Enrique’s Journey

The PATH website showcases one of our most inspiring success stories—Enrique, a member who was homeless when he first met his MedZed housing navigator. Through our Housing Transition Navigation Services (HTNS) program, Enrique’s housing navigator guided him through the process of securing temporary shelter and applying for long-term housing programs, all while addressing his chronic health conditions.


Enrique’s housing navigator learned about his struggles with diabetes, asthma, and hypertension, as well as his lack of access to food and clothing. By collaborating with our MedZed Community Health Navigator, Enrique was enrolled in ECM and received the healthcare support he needed. Regular communication between our team ensured that every aspect of Enrique’s health and housing journey was taken care of.

On his 62nd birthday, Enrique finally qualified for the Serving Seniors program and was granted a studio apartment. His housing navigator didn’t stop there—she transitioned him to Housing Tenancy & Sustaining Services (HTSS), continued to search for rental and financial assistance programs, and provided him with transportation, translation, and support services throughout his journey.

Today, Enrique has a place to call home, and his health has improved. He was also accepted into a pilot program that will pay his rent for 18 months and provide further advocacy support. “She has been an ally and a guardian angel. Now I am happier and have a place of my own to sleep, and my health is better,” Enrique said, reflecting on his journey.

This success is not just about Enrique's transformation; it's a reflection of the incredible work our Community Health Navigators and non-clinical support teams do every day. The passion, perseverance, and collaboration displayed by our field staff, combined with the unwavering support of those behind the scenes, are what make these life-changing stories possible.
